Os lycans (lobisomens) e vampiros vivem espalhados e não solitários, como são retratados costumeiramente. Essas mesmas raças vivem em constante batalha, apesar dos lycans estarem em menor número por serem caçados pelos vampiros. Uma dessas caçadoras é Selene (Kate Beckinsale), que os mata como vingança do que fizeram a sua família quando pequena.
Apesar da guerra ter sido vencida há muito tempo, as batalhas continuaram. A vampira Selene resolve investigar seus inimigos imortais e descobre que eles estão atrás de um médico humano, Michael Corvin (Scott Speedman), assim ela também desconfia que Kraven (Shane Brolly), o atual comandante do Coven dos vampiros, esteja planejando uma conspiração com Lucian (Michael Sheen), líder dos lycans, contra Viktor (Bill Nighy) o verdadeiro comandante do Coven que está em hibernação.
Além desse principal conflito, aparece o triângulo amoroso dos vampiros, como a ambição de Erika (Sophia Myles) pelo Kraven, querendo ser sua rainha e ter poder no Coven, mas infelizmente ela fica para trás por causa da obsessão dele com Selene, que o rejeita e acaba se apaixonando pelo humano.
The movie opens in the year 1202 AD with an army led by three vampire elders (Markus, Viktor, and Amelia) arriving at a Werewolf ravaged village. After killing several werewolves, Viktor and Amelia capture their target: Markus's brother William Corvinus, the first and most powerful werewolf. Viktor orders that William be imprisoned in a secret location forever.
The story then continues the present-day timeline, after the events of the first film. Selene takes Michael to a safe house so that she can return to the mansion to confront Kraven. She knows Kraven intends to kill Markus and plans to stop him. However, Singe's blood has already awakened Markus, turning him into a hybrid, who then wipes out Kraven and his men, and destroys the mansion. He learns Selene and Michael's location from camera surveillance of various safe houses and leaves to track them down. Markus confronts Selene and Michael, who evade him and hide in a warehouse.
Lorenz Macaro, an elderly and imposing man, sends in a team of "Cleaners" to investigate the aftermath from the final battle in the first movie. When Macaro examines Viktor's body he finds a metal disk attached to his ribcage which is the match to Sonja's pendant, which is in the possession of Selene and Michael. Now knowing that the pendant is of some importance to Markus, Michael and Selene set out to solve its mystery. Selene recalls that she'd seen it as a child, but doesn't know its significance. To find answers, they travel to the hideout of the exiled vampire historian, Andreas Tanis.
Selene and Michael confront Tanis, who reveals that Markus, not Viktor, was the first vampire. One of the two sons of Alexander Corvinus, Markus was bitten by a bat and became a Vampire; his twin brother, William, was bitten by a wolf and became a werewolf. The first werewolves created by him were entirely animal and unable to take human form again. Due to William's destructiveness, Markus approached Viktor, a warlord dying of old age, and offered to turn him and his army into immortal vampires in exchange for tracking down and stopping William. Markus intended to capture his brother and tame him, but Viktor ordered that William be locked away forever, far away from Markus. Viktor does not kill the brothers because he believes doing so would result in the immediate extinction of all other vampires and his lycan slaves. Tanis also reveals that Selene's father was the architect who built William's prison and that the pendant is a key, that after Lucian's escape Viktor killed Selene's family as as they knew of the prison's location, but turned Selene into a vampire. Tanis then refers Selene and Michael to Lorenz Macaro for help. Shortly after they leave, Markus arrives and questions Tanis. Tired of hearing Tanis's lies, Markus learns the truth by drinking his blood, killing him.
Selene and Michael go to see Lorenz Macaro, who is actually Alexander Corvinus, who has devoted his life to containing the Vampire-Lycan War away from the mortal world. However, Alexander refuses to help kill his son. When Markus arrives, he apparently kills Michael and learns the location of William's hidden prison by drinking Selene's blood. He then attacks and mortally wounds his father, thereby obtaining the other half of the pendant. On Alexander's bidding, Selene drinks his blood, which will give her the power to defeat Markus. Afterwards, Alexander blows up his ship.
Selene leads the cleaners to the prison to confront and destroy Markus, but he has already freed William. A battle ensues in which William bites the Cleaners and begins turning them into Lycans. Michael, presumed to be dead inside his bodybag, suddenly regenerates and joins the fight in his Hybrid form. Michael ultimately kills William, while Selene kills Markus by pushing him into the spinning blades of a crashed helicopter. It is then revealed that Selene can now stand in the sun. As the film ends, Selene narrates that she fears the days ahead, as the death of the Elders will cause nothing but chaos; but she is nevertheless hopeful.
The film tells the story of the origins of Lucian (Michael Sheen), who was the first werewolf able to take human form - to be called Lycans. Viktor (Bill Nighy), the ruthless elder of the vampires, took him home after killing his werewolf mother in a raid. Viktor envisions a race of werewolf slaves for his vampire clan that, conceivably, could keep guard during the daytime hours and toil for the vampires. As Lucian grows up at Viktor's home, his young daughter, Sonja (Rhona Mitra), develops an attraction to the young Lycan, and Lucian becomes fond of Sonja as well.
It is revealed that Viktor has a tacit relationship with the human population of the area, whereupon he agreed to protect them from the wild werewolves that roam the countryside in exchange for a tithe. As a collection of human nobles come to meet with the vampire council for protection from the werewolves in exchange for this aforementioned tithe, Sonja goes out to guard them. Lucian hears the Lycans near Sonja and steals a horse and sword from a nearby Death Dealer. Wild werewolves proceed to attack and kill the nobles, causing Lucian to engage in a battle with them. When Lucian takes off his collar and changes into a Lycan in order to protect himself, the wild werewolves back down to his howl and leave before Lucian is shot down by Viktor's men. Viktor, despite grudgingly acknowledging that Lucian saved his daughter, notes that Lucian still transgressed the laws of the coven by taking off the collar that kept him in check (by preventing him from changing into a Lycan) and feels betrayed by what he thinks of as his favorite pet. Subsequently, he locks Lucian away after having him whipped thirty times with a three pronged chain-bladed whip.
Lucian escapes, kills several of the Vampires and begins to liberate the other Lycans from what he perceives as their unfair imprisonment. The guards are alerted and start to slaughter the Lycans. When Viktor discovers that Sonja has an illegal relationship with Lucian, he imprisons his daughter. Lucian learns about Sonja's imprisonment and sets out to rescue her. On their way out they are attacked by the Death Dealers. Viktor is also involved in the chase and when it comes to the fight, it is Sonja who confronts Viktor. When Sonja discloses to him that she is pregnant with Lucian's child, Viktor overpowers her with a knife to the throat and imprisons both her and Lucian with renewed hatred. Sonja is sentenced to death for her crime against the coven at a trial presided over by her father and the Council. After Sonja's death, which Lucian is forced to witness, Lucian attempts a further escape. Failing, he is able to verbally summon both the wild werewolves and the freed Lycans, who attack the coven and overwhelm it after a fierce battle.
Finally Lucian faces Viktor in a fight and they battle their way to the dungeons. The fight continues back and forth until Lucian knocks Viktor down a well. Lucian tears at chains hanging from above, making a hole in the roof and allowing sunlight in and cornering an enraged Viktor in the darkness. Viktor's 'final' words attest to his regret at not having killed Lucian the moment he was born, to which Lucian sarcastically agrees before forcing his sword through Viktor's mouth and head and letting the body fall into the water below. Lucian declares this victory as only the beginning of what will surely be a war between the races, one bred for slavery and the other born into privilege. In one of the closing scenes we see Tanis leading a very alive Viktor, who survived due to his status as an Elder, into a hibernation chamber on a boat.
The film ends with the opening scene of the first Underworld, with the audio from the scene where Kraven tells Selene that it was Viktor who murdered her family, rather than the Lycans. That Viktor spared her because she reminded him of Sonja, the daughter he condemned to death. The last word is played over the shot of her eyes: "Lies."
